Apply to be a Wholesaler for our Health Care Provider Program

Are you a current health care provider with an active practice and experience using iodine and the support supplements?  If you are interested in integrating iodine testing and supplements into your practice, please complete this form.  In the "Account type" drop down menu, select wholesale.  In the blank box below "Phone", add your experience supplementing with iodine.  Additionally, please email your credentials.

Typically, accounts are approved within three business days. You will receive a welcome email and general information. Once approved, you may begin ordering through the website using your email address and password.

FAQs:

• Free shipping is offered on orders over $500
• Minimum order for supplements is a case (12 bottles mix/match).  We reserve the right to reclassify an account that orders below this minimum.
  
• Kits can be ordered in any quantity.   The name of your practice will be included on the requisition form.  The practice can receive patient results via fax, mail, or email.
  
• If one kit is ordered, the entire wholesale cost of the kit plus shipping will be charged.
  
• If two or more kits are ordered, an upfront charge of $10/kit will be charged.
  
• Once the patient's sample is received in the lab, the balance will be charged.
• Drop shipping is available
• New York and California health care providers may order supplements, but not testing kits. No testing kits may be sent to New York or California.  If a requisition form contains an address from those states, the lab techs will reject it.
  
• Newsletter coupon codes are not valid on health care provider orders
